Before you begin, please review the Expense Guidelines for Faculty and the 2026 Summary of Faculty Benefits to determine if you are eligible for an honorarium and/or reimbursement and what items are eligible for reimbursement.
If you are a speaker for Discovery 2026, please instead review the Discovery 2026 Summary of Faculty Benefits
At the bottom of the form, please upload the following items in PDF format :
- Itemized receipts for eligible expenses you are requesting to have reimbursed (receipts are required for every expense)
- Check the newly revised Expense Guidelines for Faculty to confirm that all expenses are eligible for reimbursement.
- If you are eligible to receive an honorarium, submit a completed
W-9 form (or W-8 form if you reside outside of the United States) - If you reside outside of the United States, please upload wire transfer information
