This form is designed to allow practices or institutions to pay for multiple registrations at once. Each person for whom you are paying should have registered separately for the activity and indicated on the payment screen that they will pay by check. Once this bulk payment is processed, each person's order will be cleared with a note indicating actual payment information. 

Please read the instructions and complete the form, below.

1. Log into your AAAAI account by clicking "Log in" in the top right corner of this page.
   1. If you need help logging in, please click HERE to be taken to our Help Center.
   2. When you are logged in you will be redirected to this page.
2. Complete the form, below, for the first person on your list.
3. Click 'Add to cart'
4. Click the title of the activity on the cart page to return to this form to enter additional attendees.
5. When you have entered all registrants, click 'Check out' on the cart page.
6. Enter your information and select payment type.
7. Review and submit your order.
8. Complete the payment step (via PayPal, credit card)
