Asthma and food allergy are common conditions managed by allergist/immunologists, and new therapy is welcome for both conditions. Regarding asthma, the relatively recent introduction of biologics has revolutionized the care of patients with severe asthma, but not all patients with severe asthma improve with biologic therapy. The first study explored the relationship between mucus plugging on chest CT scans and responsiveness to biologic therapy. The study showed that higher degrees of mucus plugging correlates with worse pulmonary function at baseline but also predicts a larger clinical and pulmonary function response to biologic therapies in severe asthma. These results suggest that the degree of mucus plugging can be useful in the selection of candidates for biologic therapy among patients with severe asthma.
Regarding food allergy, peanut allergy is a particularly common and negatively impactful example. Although the classic treatment has been avoidance, the ubiquity of peanut in the diet makes avoidance difficult, and accidental peanut exposure is common and often results in severe reactions. More recently, oral desensitization and omalizumab have been approved for the treatment of peanut allergy, but both treatments come with drawbacks in young children. The second article describes the long-term follow-up of an another form of desensitization, epicutaneous immunotherapy. The study found that after 24 months of epicutaneous peanut patch immunotherapy started in children 1-3 years of age, there was continued enhancement of the treatment effect without additional reports of treatment-related systemic allergic reactions, demonstrating the potential of epicutaneous immunotherapy to be a safe and efficacious treatment option for the management of young peanut-allergic children
The thorough discussion of the strengths, limitations, and clinical applicability of these articles by experts in the field that is provided in the Virtual Journal Club format should allow the listener to use the information in these studies most appropriately in their clinical practice.
