If you will be presenting in an Annual Meeting session scheduled for recording, we need you to submit one or two multiple choice questions that review key concepts covered in your presentation.  All Virtual Joint Congress questions must be submitted through this form by February 19, 2018; no exceptions will be made.

Questions should be multiple choice with a single best answer option provided. Case based questions are especially helpful for learners. Including a rationale for the correct answer is also a helpful learning tool.

Questions will be presented exactly as they are submitted in this form. If you are presenting in more than one session that will be recorded, please complete a separate submission form for each session. If you would like to submit more than two questions for a single session you may complete this form multiple times.
