Design:  We started with asking asthma patients to set goals that were self-selected. We have reviewed the first round of goals, with plans to improve how we collect goals, and identify how we as a practice can help patients meet these goals.

Main Findings:  We found that patients set both short and long term goals, and that most goals are focused on function.

Impact on Practice:  We are refining the goals collection sheet for the 2nd cycle of goal collection. With the 2nd cycle, we will identify which goals are not met, and how we can change our practice to help patients achieve their goals. We then have a plan for a final cycle to see if we can help patients achieve their asthma goals 75% of the time or more.
